Issuance of a permit (licence) performance of cremation

1. Service requests
In order to receive the service, the local government shall submit:
1) an application (see the form in the Annex to Regulation No 600 on Prophecation Rules of MK 29.06.2010);
2) documents attesting the right of a merchant to use immovable property (buildings) for the performance of cremation, if such immovable property is not the property of a merchant;
3) documents attesting that in a building in which a merchant will perform cremation, there is a separate room in which there is a special kiln for the cremation of the body of a deceased person (hereinafter - dead); a room in which the deceased is prepared for cremation and a place for the storage of the deceased at a temperature not exceeding 4 °C; a separate room in which farewell-bye is provided for the deceased;
4) documents attesting that the referred to premises and the room in which the merchant keeps the proceedings are equipped with an alarm and access control system and are ensured against unauthorised entry of unauthorised persons;
5) a copy of the document certifying payment of the State fee or a printout of the payment made through the internet bank, in which the name, surname or name of the payer shall be clearly legible and which has been signed by the payer. If the payment has been made through an online bank and the payment document performed has been signed with a secure electronic signature and conforms to the requirements specified in the regulatory enactments regarding the drawing up of electronic documents, the merchant may send it to the official electronic mail address of the relevant local government.
At the same time as the submission, a merchant may also submit (not older than one month) a statement of the State Revenue Service regarding the fact that the merchant does not have tax debts, information from the Regional Bureau of the State Environmental Service regarding whether the merchant has received a Category B polluting activity permit.

2. A service charge
The State fee of EUR 113,83 shall be payable for the issuance of a special permit (licence). A national fee of EUR 14,23 shall be payable for the issuance of a duplicate of a special permit (licence) or a repeat special permit licence. The State fee, in accordance with the provisions of Regulation No 600 “Kremlin rules” of BOM 29.06.2010, shall be included:
